Comprehensive Assessment and Plan of Care

The comprehensive assessment and plan of care continue to be cited in the top 10 deficiencies by Centers for Medicare and Medicaid Services (CMS) surveyors. This NHPCO Webinar offers a new approach and fresh strategies to transform your documentation of the comprehensive assessment and care planning processes. This is done by drawing on the expertise of the entire interdisciplinary group (IDG). Regardless of patient age, hospice providers must develop an individualized plan of care that can be reviewed and updated based on the patient’s ever-changing condition.
